When you use a participating East End Health Plan provider your benefits are paid at 100% after a co-payment of $20 in most cases.
In some cases, there is no co-payment necessary.

All payments are subject to reasonable and customary (R & C) amounts.
First, you must meet the individual calendar year deductible of $1,000 or the maximum family deductible of $3,000.
