Services
When you purchase a health insurance policy, you agree to pay a certain amount each month to the health insurance company.
This payment is called a premium.
In return, the insurance company agrees to help you pay your medical expenses, such as bills for medical services that help keep you well (like flu shots and checkups), for treatment you need when you are sick or injured, or for prescription drugs.
Under the Affordable Care Act, the overhaul of the health insurance system enacted in 2010, the federal government may help you pay some or most of the premium.
A "surprise bill" can also result when an in-network doctor refers an insured person to an out-of-network provider - for example, by taking a blood sample from the patient in the office and then sending it to an out-of-network laboratory - without the patient's approval.
The new law also protects consumers from bills for emergency services.
You have coverage with a health maintenance organization (HMO) or an insurer subject to New York law.
You are uninsured or your employer or union provides self-insured coverage that is not subject to New York law.
Inpatient care (care in which you're formally admitted to a hospital with a doctor's order).
Mental health and substance abuse disorder services, including behavioral health treatment, counseling and psychotherapy.
Services and devices that can help you recover after an injury or that you need for a disability or chronic condition.
Preventive services (such as screening tests, vaccinations and contraception*) and help with managing chronic disease.
Pediatric services, including dental and vision care for children until their 19th birthday.

Claims must be submitted within 120 days of the date of service for prompt adjudication and payment, unless otherwise noted in your provider agreement.
